Job Description

About the Role

Reporting to the Commonwealth Psychosocial Support (CPS) Team Leader, this role is responsible for providing psychosocial support and assisting clients in accessing the NDIS. This is a non-clinical community-based supports that aim to facilitate recovery in the community for people experiencing mental illness – through a range of services to help people manage daily activities, rebuild and maintain connections, build social skills and participate in education and employment.

Other responsibilities include:

  • Developing individualised recovery plans that facilitate choice and control by the individual
  • Linking clients to other services, supports and community networks as required
  • Supporting clients in navigating the NDIS application process where appropriate

This opportunity is 60 hours per fortnight, working across Monday to Friday.

Working hours are generally between 9 am to 5 pm.

Travel: You will be required to use your vehicle to travel to clients/participants & will be reimbursed kilometres between clients and travel between clients will usually be up to 30 minutes, with occasional exceptions.

 

 

About You

To be considered for this role, you will possess;

  • Qualification in Human Services (e.g. Mental Health, Community Services, Social Work, Psychology, Counselling)
  • Knowledge of NDIS practices and application process
  • Demonstrated experience in mental health work and a sound understanding of recovery within a human services context.
  • Knowledge and understanding of the spectrum of challenges that people face in accessing and maintaining links to health services
  • Demonstrated experience in developing and maintaining positive relationships with a wide range of internal and external stakeholders including clinical services

  • NDIS Worker Screening Check 
  • Current Driver’s licence
